I am trying to get E-APO to work on Windows 7 x64 with Realtek HD Audio Manager currently installed.
Upon installation and restarting no Config folder was created in Programs\Equalizer APO 0.9.1 nor was there any config.txt file.
So I created both (config.txt inside the folder). Five items were initially in the Equalizer APO folder: Benchmark, 2 online links, Configurator and Uninstall.
Placing various settings in config.txt and saving produces no changes in my system sound, which does continue to work (such as "Preamp: -60 dB").
After installing, the Enhancements tab under Speaker - Properties is completely blank, so it is not possible to
deselect anything and no button is available under Speaker - Advanced for Enhancements.

As the config folder is missing in your installation, other files might also be missing that are required for E-APO. Please try to uninstall und do a clean reinstall.

Maybe you should try by (re)installing the latest drivers for your Realtek HD Audio codec ... normally you find the latest drivers through your motherboard manufacturers support pages (generic drivers can be downloaded from Realtek's downloads).

Yay! Downloaded and installed the latest drivers from motherboard mfg. After several restarts (uninstalling E-APO, reinstalling...) the test change to the text file worked!

Well.. I then installed Peace.exe into the config folder, started it and now E-APO is not working; either with the GUI interface or if I go into config.txt and change it (adding "Preamp: -60 dB").

I don't know the Peace well (developer Peter is active here) but if there's Include: line added for to load Peace configs in config.txt then just try by commenting/deleting the Include: line.
Do what Juha is suggesting: delete the include. And then add the preamp. If E-APO isn't responding, it's not Peace related.
If E-APO is responding and keeps on responding well to commands in config.txt, it is Peace related. In that case I can try to help you.
As Juha said, the .log file can contain some clues why things aren't working.

You stated that you checked "Disable all sound effects"...
This will also disable EqualizerAPO, at least this is the behavior on my system.
But to make it more complicate, it doesn't disable it immediately after checking the box or clicking "Apply", but when the playing source is stopped and then startet again.

Input format is not supported by child apo. To get exact information of values used there, you need to enable the tracing feature as instructed on wiki.

Looks like there's initalization issue (if that's all trace log you got). Delete the log file and then restart the computer to see if the initalization issue gets fixed. Compared to trace log from my other PC your's was little too short. You could try by changing the playback samplerate to say 24/48000 (through audio control panel) and then paly some audio.
Jonas knows best this type issues so if it didn't got fixed by that then you have to wait his appearance.

I recently had a similar behaviour on Windows 8.1 with the newest Realtek driver. Seems that the same problem can also occur on Windows 7. There seems to be a crash in the APO while it is negotiating for the input format.
For some unknown reason, the Realtek driver seems to install in different ways when you reinstall it. I could see that by the entries in the registry.
So, my advice is to uninstall and reinstall the Realtek driver, maybe multiple times, and see if you get it working again. On Windows 8.1, I really had trouble to reproduce the problem you are facing on Windows 7 now, which is why I could not yet analyze the problem with a debugger. But for you, this means, that there is a high probability that E-APO works again when you reinstall the driver.

May I suggest that after you get the EqAPO working again, try some of my GUI's listed @ http://jiiteepee.tripod.com/home.html . Remember install those needed components as well (C++ Redistributale for VS 2013 & .NET 4.5).

I am working on it. So far no luck. I have uninstalled everything and reinstalled - have E-APO showing up fine; but even after many re-installations of the Realtek driver and restarts it is still not working.
I also did a system restore to a point a couple of weeks ago- - to bad I didn't create a restore point when I had it working.....

(- I'm not sure if it's needed to uninstall EqAPO before above process (I would uninstall)).
- reboot
- uninstall realtek related drivers/software
- reboot (after Windows is up again you probably get message regarding new hardware found but don't install anything just shut it down)
- start computer again
- when system messages it found new hardware don't let system do the installation as it suggests (skip the installation process)
- run the realtek driver installer
- reboot
(- test if audio works)
(- make restore point)
(- install EqAPO and set it up as instructed on Wiki)
(- test if EqAPO works (preamp, some filter commands)
(- make restore point)
(-install GUI software)
(-test if everything still works)

Does it install automatically even if you disable the realtek through device manager after uninstalling its drivers/software (after disabling, reboot twice, then enable the device through device manager and finally install drivers by running the realtek installer?

I don't think that I can disable the realtek if I have uninstalled it through the device manager since it will disappear.
I did find out that there is a system driver cache which holds an old version of the realtek driver and which reinstalls it even with no internet access.
Maybe I'll disable then uninstall it; but I bet on restart it will resinstall.

Normally, you just need to run the Configurator again and select the audio device after reinstalling the driver.
OK, so I ran the configurator (a few times), restarted the computer, and the config.txt file was working (Preamp: -60 dB).
Then I took Peace.exe, put it in the config folder, opened it, and - nothing. Connection severed.
Deleted all files in Config folder, recreated config.txt, still nothing.
Now I am going to get back to stage 1 having E-APO working by itself.
So far ran the configuartor many times and restarted a few times and it remains not working...
