I ended up using Cleanrip to capture ISO files from my games, and an older build of USB Loader GX (V 3.0 r 1268) to launch. It works, but the game images are big.

Ran into this in 2019 when softmodding a black Wii (RVL-101, without the Gamecube Ports) that I saved from going into a landfill.
I am using a Crucial BX500 120GB 3D NAND SATA I got from amazon for $22 that I had laying around. I don’t have a lot of discs so it should be enough for now.
Also got a cheap $14 enclosure when I bought the drive. It’s working great for a while on both my systems so I figured I'd format it and try it after my USB stick wasn't working.
Followed all the guides and made sure to install beta2 of d2x (not beta3-alt like some infamous guide photos show) but I was still seeing the same inconsistent failures when installing from discs.
I did some more digging and decided I needed to try some other cIOS based on suggestions from other threads that got closed here. This isn't a support forum, but it shows up in google when you search for the issue so here we are.
More digging eventually lead me to this guide , where they show installing slots 249, 250 and 251 https://wii.guide/cios.html
After that I launched USB Loader GX and went into the Settings -> Loader then scrolled down to where it shows the “Game cIOS” and changed that number to 251.
It worked for me at that point. I am not sure if the version of Wii mattered (again mine is a black, NTSC sometimes called "family" edition post-2011 RVL-101) so much as the right HDD and cIOS combination?

had the same issue, downloaded the previous version and all was ok. the newest version did seem to operate normally to play games from hdd etc but would just not save any games to hdd.

Was still running into some issues so I tried Michael's suggestion and it did allow me to back up a smaller (300mb) game. Other games were still freezing up even though the disc was still spinning. I tested another theory. Backing up a dual layer 6gb game (you know the one) and it dies at 20%. I decide to just leave it, walking away in frustration from my retro tv setup. This morning I come back downstairs and hard reset the Wii. To my surprise: there's the game, backed up perfectly. I have no idea how long one must wait (probably not all night) - but if you're patient, you can probably safely just let it "fail" like that then come back an hour or so later, reboot and see what's actually there. More and more I feel like this is a problem with USBLoader's progress bar/indexing than it is the process that actually rips the DVD?

I had the same problem too. About 1 out of 6 disks worked. I downloaded a previous version, all are ripping nicely. Not sure why. (first tried r1272 had problems. downgraded to r1271 working great)

same issue with usb loader freeze during game install on r1272. Downgraded to r1271 and still had freezing.
Downgrading to r1268 solved the freeze problems for me.
