#173 Exiting submachine state through an eXit-Point Segment fails to execute exit action from submachine

If a submachine has an exit action, it should be executed on every possible transition out of the submachine. However, in some state machine topologies, an exit action fails to get executed when a submachine state is exited through an eXit Segment. Specifically, this happens in QM 4.0.1 when the corresponding eXit Point inside the associated submachine is reached via a transition that originates on a nested state, as opposed to transition originating on the submachine boundary.
