Increase version to 0.9.3 (forgot to upload this after the 0.9.3 release)

Fix missing check for channel<0 in VIDIOCGCHAN

Updated the "Optimizing the stream" added: "Scaling and offset correction" and "Frame rate conversion"
And some other small fixes

Good idea, couldn't be made to work. *severe* artifacts, and after a week
of trying 3 different methods all that was accomplished was slowing it down
about a factor of 5. Anyone wants it I have the sources of the 3 attempts.

Add some code inspired by some Iomega magic

Add safety comment

* Fix LML33 problems for NTSC too (guessing - I don't have NTSC)
* Add autodetect norm for LML33/bt819
* make zr->card a struct instead of a pointer so it can be modified per-card

Change default threshold to 5% (probably should be 0 or removed completely)

Fixing LML33 PAL video decoder setup (well almost...)
