[Mantisbt-dev] Filters by Date and Custom Field

Attached Patch will allow Searching by Date and Custom Fields.

Patch Changes:
- Enables Date Selection HTML
- Adds Filter Logic for Date Filters
- Adds Filter Logic for Custom Fields
- Adds Custom Fields to Cookies
- Adds Custom Fields to Filter Form
- Adds Config Var to Enable Custom Field Filters
- Adds Config Var to Set Number of Custom Fields Per Row
- Bumps Filter Cookie Version
- Includes a few MySQL Table Optimizations

At Cyan Worlds( http://www.cyan.com ) we have been using this for 
several months and it has worked pretty well.  Our Mantis Site is 
completely private, so there could be some security improvements to this 
Patch, but I think these are important missing features from Mantis.
