One common reason is that paths are not correct. And don't forget to make config.xml readable by Apache, and factory.sock readable and writable by Apache. I can't tell what's causing the issue with Apache::Request. Let's try to fix that "libmacs context uninitialized" message (which means the Apache module either couldn't read the config.xml or couldn't contact the MACS servers) and see if that fixes the Apache::Request problem as well.
