I don't think this problem has been reported here from what I can see but Intuos 2 tablets don't work if hot plugged, they only work if they are connected when X starts. The relevant part of the x log seems to be;

(EE) Wacom Intuos2 4x5: usbParse: Ignoring event from invalid serial 0

there is a more detailed bug report at https://bugs.launchpad.net/ubuntu/+source/xf86-input-wacom/+bug/532137 which as far as I can tell never actually got logged here. so I'm logging it here since I'm also seeing the problem.

    I tested both Intuos and Intuos2 with xf86-input-wacom-0.10.11. Both work as expected. Not sure when and where the problem has been fixed. But it is fixed. So, close the bug now.

    I am experiencing this issue, on a Wacom Intuos 2 9x12 connected to a machine running Fedora Core 18. However, I can only trigger the error messages by disconnecting the tablet and doing an rmmod wacom, and then plugging it in.
    wacom x11 driver version 0.16.1.

    However no matter if I do the rmmod or not it won't change cursor position or the like.
    Not doing the rmmod has the light on the tablet always in the orange state, but doing an rmmod restores it to what it should behave like - it blinks green whenever a stylus button/tip/eraser is used, but no reactions from X apart from the error above about usbParse.

    Oh, and it works as expected if it's connected at boot, or if not connected at boot and connected for the first time. This only appears after disconnecting it once and then reconnecting.

    I'm also affected by this bug - running Linux 3.11.0 with xorg wacom driver 0.22.99.1-1+b1 from debian unstable. I came to the same conclusion: rmmod, unplug, replug -> green LED + log messages; unplug, replug again: LED stays red, no error in log. In both cases, the device is detected just fine, as noted in the Xorg log.

    I once got it working by rmmodding, but that must have been luck...
