Can you tell me more about your document-centric approach? I think I don't
understand all the meaning.

I don't think that the underlying environment need to require this. This is
just
a mater of usability and end users training. Users who test this are not
disturbed
and they fill this more logical.
Avoid File->Quit menu in all open source projects, replace it with
Application Menu
as I described and you will see it at as a new GUI standard in a few month
(or year).
