- Implementing an easier-to-use VCF file reader API.

- Adding an option to not require indexes on VcfFormatConverter.

- SAMFileReader.java: Modify SAMFileReader.streamLooksLikeBam to accept URL.

- MakeSitesOnlyVcf.java: Bug fix: if SEQUENCE_DICTIONARY is null, NullPointerException is necessarily thrown.

- ReadNameFilter.java: eliminate Scanner class usage to reduce memory footprint.
