When you attempt a punch using a 'Mech that was built without hands and lower arm actuators, MegaMek is giving +1 for the missing hand as well as +2 for the missing lower arm. For example:
To Hit [RA]: 6 (72.2%) (1 (base) + 2 (attacker ran) + 2 (Lower arm actuator missing or destroyed) + 1 (Hand actuator missing or destroyed))
Damage [RA]: 4 (using Punch table)
and/or
To Hit [LA]: 6 (72.2%) (1 (base) + 2 (attacker ran) + 2 (Lower arm actuator missing or destroyed) + 1 (Hand actuator missing or destroyed))
Damage [LA]: 4 (using Punch table)
According to Total Warfare page 145:

Missing Actuators: A ’Mech does not need hands (or hand
actuators) to punch. However, ’Mechs not equipped with a
hand on the punching arm must add the +1 modifier as for a
hand actuator critical hit. Likewise, ’Mechs that do not come
equipped with a lower arm actuator on the punching arm
must add a +2 modifier to the to-hit number (in place of the
+1 modifier for punching without a hand actuator), and the
punch inflicts only half the standard damage (round down).

Specifically notice the part that says, "...’Mechs that do not come equipped with a lower arm actuator on the punching arm must add a +2 modifier to the to-hit number (in place of the +1 modifier for punching without a hand actuator)...". Thus, for 'Mechs that are purposely built without lower arm actuators, the +2 for the missing lower arm replaces the +1 for the missing hand. The two modifiers are not cumulative.
Checked the errata and didn't see anything related. It looks like, according to TW pg 126/127 that actuator damage is cumulative, but that missing actuators are not.
