This means that the timer thread didn't set the event to unblock the thread
init thread.

Are you seeing the same behavior with the pre-compiled libusb-1.0.19?

Would you be able to add the following lines to further debug? Line numbers
are taken from current file without modifications.

windows_usb.c, line 1028: usbi_dbg("starting timer thread, event %p",
event);
windows_usb.c, line 1037: usbi_dbg("waiting for timer thread to be ready");
windows_usb.c, line 2341: usbi_dbg("timer thread started, param %p", param);
windows_usb.c, line 2345: usbi_dbg("timer thread setting event");
