That would arguably be a better way to do it, marking the words according 
to their function rather than typographic characteristics.  I would
point out, however, that the current python parser would not let you
nest one interpreted text within another.  With the Perl version, you could
say::

  Kook, Loony. :book:`Johnny says, ":foreign:\`Gesundheit\`!`

where the backslashes enable the outer interpreted tag to find its
correct closing tag and then get removed before interpretation by the
parser in the nested context.
