Summary: v1.39 infinite loop w/inherit keywords

Initial Comment:
With inherit.keywords !=0, there seems to be an 
infinite loop in html/dockbook.xsl:141.

xsltproc catches the loop at a depth of 250!

the following is an edited walk through the beginning 
of the loop - write.chunk->chunk-element-content-
>html.head->head.content->*->*

It goes away when inherit.keywords is disabled.

I think this is an XSLTProc bug. Looking at the code, I
find:

  <xsl:if test="$inherit.keywords != 0
                and parent::*">
    <xsl:apply-templates select="parent::*"
mode="head.keywords.content"/>
  </xsl:if>

This can only loop infinitely if parent::* returns true when
the context node is the document element. It shouldn't.
