The toc.ncx file does not correctly reference the index.html file. The file: my-epub.xml is the same as the file: my-epub2.xml except the book tag DOES NOT have an xmlns attribute. The toc.ncx file has a correct reference to the index.html file. I have included the script I used to get these results and and resulting files. I shamelessly adapted examples by Liza Daly. Everything I know about Docbook -> ePub I learned from her examples. Also, could you kind tell me what (free) Unix tool I should use to validate my Docbook 5 documents? The Docbook 5.0 XML source file was validated with relames. However, the generated ePub files does not validate with epubcheck 1.0.5 Please let me know if you need any more information.
