Patch to allow building against FFMPEG4

Attached is a patch to allow VICE to build with current FFMPEG libraries (4.x). It needs to be adapted to support building with earlier versions including the one in the source tree I expect, but this appears to be the minimum required for 4.x.
I always build using --external-ffmpeg, which seems to depend on libavresample-dev etc. And that works fine, ignoring the shitload of deprecation warnings.
What does this patch do exactly?
Applied, with some extra code to support older ffmpeg versions (incl. our in-tree ffmpeg) in r35787.
Works on Ubuntu 18.10, not yet tested on other distros, or Windows/OSX. So some additional testing might be required.
