If you still have problems then would be useful that you describe all the steps you did and to have a copy of the image that is causing the problem, also the versions of OS and Tux Paint you run. Note, I've made some quick tests (on linux adding a starter to my home directory) and see that once a starter's thumbnail is created it is no more updated even if the starter changes. That could lead to the behavior you described if one adds a starter, tests on tuxpaint, then modifies the starter to improve it but for whatever reason a blank image is saved in its place: the thumbnail on the New dialog will continue to show the original image but the blank starter will be loaded.
