Let the user leave and optional one line comment when editing wiki pages. Closes #62.

Added support attaching multiple files with the same name. TRAC_ADMIN can
now delete attachments. This should finally close #27.

Meta permissions should also be added to the permission cache. (reported by Laurie Harper).

Fix the view as text/raw links for attachments.

Added smtp_always_cc to always send noitification to a specific list of
addresses. This is useful for sending issues to a dedicated mailing list.

Prioritize silvercity before enscript, for better performance.
