Use RFC:2396 style scheme also for InterWiki prefixes.
Should fix trac-ticket:... and trac-changeset:... issues seen on Trac hacks reported by athomas.
Also improved a bit the output of the ![[InterWiki]] macro and did some refactoring for the expansion of link arguments.
