There was a similar question last week and reply should be in archive. Basically, there is some known bug that occurs at random times like you describe in SoX's OSX audio driver but so far I'm unable to debug it (or reproduce it). I'm hoping an OSX developer will step in soon that is more capable at debugging the issue.
