#21 patch for hydra/ettin blindfolding

Ettins and Hydras will no longer be blinded by wearing a blindfold on their 'head' and if the player has intrinsic telepathy the blinded head will enable the player to also see telepathically.

Adds a new monster flag, M3_MANYHEADS for monsters with multiple heads.
