It would be helpful to see the syslog output from the moement when the NPIV target ist activated. Have you checked the initiator end using QLogic SanSurfer tools? How's your physical setup - ini with multiple dual-port cards (which ports are connected), probably one/two FC switch(es) inbetween, what's the target like (probably again a 2562 - single or dual connection?)? Any zoning active on the SAN switch (if present)? What's your NPIV's group definition on the target?
