This seems to prevent getting 100% kills on maps with rotfish. Is this possible to fix in the engine, or is it just a bug in the iD pak files?

This is just a quakec bug with id pak files, and not an engine bug.
Closing as wontfix.
Even though this was marked as wontfix, I wrote a patch to work around this problem. The fix was ported from the reQuiem engine. I don't know if you want to apply it or not, but maybe it will help some user at some point.
