You can even see this bug with the naked eye - turn on the automap with overlay and iddt-iddt and you can see that the player gets stuck in a sergeant (ok, I have the advantage of the chasecam in the dev version to help see this :). It turns out I'd failed to completely fix the mess of overlapping uses of global variables in p_map.c - utterly horrible piece of code. Boom added several extra overlapping uses of some already very overloaded variables, and this was breaking the telefragging code when more than once monster was telefragged at once (two sergeants in the case of E4M1).
