When an image reference like for a user or a product is removed the image remains in the table posper_image but can never be reassigned.
Product images cannot be reused either.
There is a working user interface to handle images in POSper Backoffice which solves the issue described in the original bug report and which also provides the possibility to retrieve images through a URL from a cached web site. We'll eventually implement an option in POSper to retrieve imges from a URL rather than from the database.
