There doesn't seem to be any pattern to it, and most of the time there is no warning. It seems to make no diffence if you say no, or if you say yes. If the OS does reinstall, nothing is different. The NSI consensus is that this is part of UAC in Vista. Although there is not a consensus on the exact cause, all seem to agree, that clicking either of the two options produces the same result. Running the installer again will produce no warning. The fix is to add RequestExecutionLevel. Most people seem to think it comes from downloading the installer from the web. The OS marks the file as from an unknown source and then puts up the warning message when you execute it. Whether that is right or not I don't know. But, I found I could reliably reproduce the warning by putting an install package on the build machine, download through a web-browser, and install.
