remove some generated files that shouldn't have been checked in.

Integrate y4mscaler into mjpegtools. Now y4mscaler will benefit from the
cpu detection and optimization flag settings - i.e. it will no longer be
necessary to manually edit the y4mscaler Makefile for the cpu type, etc.

Remove dead / uncompilable code.

Some changes for the release candiate.

The patch fixes a cross compiling problem. That can happen if you don't have a /
usr/include

enable the -u (short form of --cbr) - was accidentally left out of the options
