Added: some more two-pass encoding support.
Fixed: added ceiling to bit-rate demand generated by for target bitrate
control so exceptionally complex pictures can't shift the control
bit-rate so high that the video buffer continues to empty even
if its already nearly empty.

There was another cpuid call that needed to be fixed.
Christian Ebert found it and provided the fix.

Added a fix for MAC where the compiler hat a probelm with compilinge the ASM part.

He wrote SSE2-accelerated versions of filter_plane_median()
and temporal_filter_planes()

Tuning: some minor tweaking on video buffer gain

Fixed: muchtoo large video buffer feedback gains.
Fixed: missing clamping of bit allocation (-ve allocations
could be produced which caused craziness...)

Fixed: oops clamping quantisation >= 2.0 rather than 1.0
Fixed: Less biased intra quantisation (merely rounds rather than
biased slightly away from 0.
Added: flat quantisation matrices that should hopefully be less
prone to blocka artefacts for very-low-q encoding of pictures
with little noise/very little texture.
Added: MJPEGTOOLS_SIMD_DISABLE tags for disabling SIMD fdct and idct.

Fixed: mquant predictor was being set from suggested quantization
and not the actualy quantization after possible incrmements to
avoid saturation.
Experimental: fixed some obvious oops in the look-ahead rate-controller
adjusted a ridiculous large gain in pass-1 and put in
some additional 'sanity' limits on rate-control feedback.
