Fix minor warning-generating errors in source-code.

Fix various initialisation bugs detected by valgrind in Pass-1 of
on-the-fly rate controller.
Switch on-the-fly rate controller to use the variance instead of old
DCT coefficient based 'activity metric' for rate control so rate control
can setup without all DCTs first having been done.
Fix a number of minor errors causing compiler warnings

Need to install mpeg2syntaxcodes.h and imageplanes.hh for compiling against
libmpeg2encpp.

Add to LIBLADD to work around a windows linker limitation.

Further finetuning

a) should run under OSX with Altivec-SIMD, now (Steven can you pls
check this?
b) should be a lot faster (time-recursive approach)
c) should be capable of giving the full vertical resolution ;-)
(it might however produce visable ghosts on scene changes, which
is not possible to come arround completely...)

There is no Lanzcos filter anymore...

rearranged deinterlacers internals. Trying to prepare for release...

Fix a few typos but more important remove old or incorrect advice. Many of
the options in the mpeg2enc examples should almost never be used unless the
person knows exactly what they are doing - the options should not be in a
guide used by novices!

it is a speedup when readding the same file several times:
- it does not change any functionality of jpeg2yuv.
- it does not change jpeg2yuv's command line interface.
- it massively speeds up the generation of stills, when using jpg2yuv
as shown in my first 3 examples. As most users will opt for the
first or second example, this patch will help them.
