Need <cassert> when using assert() in C++ modules.

Fixed some subtle Bugs in multi-pass/lookahead coding implementation
caused by lower-level routines member variables of Macroblock components
via side-effects.

Fixed some serious ugliness in the handling of Macroblock skipping
inadvertently introduced via refactoring.

Sigh, have to zero out the 'pasp' structure to avoid getting junk values from
the stack. This was causing the program to think it had a pasp atom when the
file did not have one.

config.h must NEVER be present in exported/public header files!!

Remove trailing / from pass1ratectl.cc.
Add "cpu_accel.h" to rate_complexity_model.cc for the fmin() prototype - some
systems don't have that natively.

Checkpoint: pass-2 now active, rate/complexity model classes added,
refactoring of seqencoder to remove nasty interactions between pass-1
and pass-2

Apparently the corruption of the edges has been fixed so the comment about
the corruption can be removed. The default for the median filters is still
0,0,0 since enabling those can result in loss of detail and thus shouldn't be
on by default.
