Added for Martin Samuelsson a patch.

You can now use now 1-10 inputs from a card. This is needed when you have a AverMedia 6 Eyes card.

Add a correct (for files with no cadence breaks) inverse telecine (2:3 pulldown
removal) utility.

The median filters corrupt the top and bottom edges. Disable the post-median
filter by setting the default to 0,0,0 (the pre-median filter was already
disabled).

The backslashes were causing the paragraph to be misformatted.

Remove hardcoded '/4' in size calculations. Accept extensions - work with other
than 4:2:0 (if anyone's actually/still using this program ;)).

Tidying things up a bit. lav2wav.h was only used in 1 module (lav2wav.c) since
2002 so inline it. Use mjpeg_error_exit1() instead of mjpeg_error();exit(1);

declare stack variables with an alignment of 16, for apple gcc which has preferred-stack-boundary=4 and would otherwise optimize out ALIGN_PTR

Support apple-darwin on multiple hosts

Oops - cut&paste left out the '-fno-PIC in the 'else' (non apple-darwin)
situation.

extend the -mdynamic-no-pic option to apple-darwin on Intel (MacIntel) rather
than restricting to PPC only. Seems yuvscaler needs this.
