This doesn't quite work yet - but rather than revert the addition of qttoy4m
in Makefile.am check the source for this program in and debug it when
libquicktime's colormodel conversion (packed -> planar) is either fixed or
figured out.

Tidied and simplified calculation of input frame index
from encoding order index and temporal reference

Fixed Bug where GOP splitting could introduce a broken
closed GOP.

Add the new executable (y4mtoqt).

Add uncompressed 4:4:4 (v308) support, not overly useful since probably nothing
except FinalCutPro will want it.

Remove debug logging statement.

add y4mtoqt for creating 2vuy (422) Quicktime files with audio track.

Complete the relocation of 10 programs in lavtools/ to the new y4mutils/

Uncluttering lavtools (in preparation for adding several more utilities) by
relocation to the new y4mutils directory

OK, this is a big quality improvement... The image quality now is better
(by far!) than by any other deinterlacer (motion-compensating or not) I
know. I should disable the lowpass-filter in the postprocessing-stage,
too (This then gives an image which nearly does not show any loss in
sharpness...)

Bugs: first two fields garbage. Need to interpolate spatialy.
