Recent experience shows that "-m 10 -M 3" produces much higher-quality
output than the old defaults. So those are the defaults now.

Speed up patch submitted by Scott Smith (Stefan Fendt was short on free time and
asked that I check the change in for him).

MMX/SSE speed enhancement and cleanup contributed by scott at gelatinous dot com

Would make sense to be consistent and use the variable set with the (correct)
limit instead of a hardwired constant.

Aieee - got the limits reversed for the Y' and CbCr. Luma <=235, Chroma <=240.
