Tidying up a bit.

Implement --cbr and fixed the --closed-gop option (it doesn't take an argument).
--cbr turns off -q (sets it to 0).

abs() is in stdlib.h rather than math.h. Thus the error:
denoise.c: In function `low_contrast_block':
denoise.c:169: warning: implicit declaration of function `abs'
was being given during compilation.

Wow - quite a few unused variables were picked up by OS/X's version of gcc.

Begin adding support for forcing Constant Bit Rate (CBR) with MPEG-2. At the
present the manpage has been updated and the option "--cbr" (-u) sets a flag
which will be used in the near future.

Fix typo - should be "bitrate" not "birate"

Why set "mpeg = 2" twice :)

Updated to reflect the new code & the new meaning for command-line
parameters.
More hints on proper usage.

The BUGS section used to say there was a need for a dynamic noise
reduction tool. We now have 2.

Changed reference to "yuvdenoise -I" to "yuvdeinterlace".
