Nicolas Boos submitted the idea/patch to allow changing individual parameters
of the filters without affecting the other ones. With -L and -C you have
to specify all the parameters and the only way to leave some values untouched
is to read the sourcecode to find the defaults. With -x/-X/-y/-Y you can
affect specific parameters and let the other(s) default.

remove mjpegtools-config, add mjpegtools-config.h.

remove redundant #ifdef HAVE_CONFIG_H block. The .cpp source modules already
do the #ifdef #include config.h _and_ exported headers should never use
HAVE_CONFIG_H or config.h

Fix APP/COM JPEG marker writeout

The new configure.ac arrangement has proven to be a success - time to clean
out all the commented out old code (which was 10% of the total size of the file
;))

it was pointed out that there were a few lingering (inconsistent) uses of
quoted #include "file" remaining. To be consistent those were changed to
#include <file>
