Fix potential integer overflow

remove the y4m_accept_extension call now that the lower level routines accept
both 420mpeg2 and 420jpeg. this program really is not prepared to deal with
411, etc data.

If feature-level < 1 (i.e., older code), then accept all/any 4:2:0 chroma
modes, not just 4:2:0-JPEG.

(The default with no 'C' tag is still 4:2:0-JPEG, though.)

Add y4m_accept_extensions(1) BUT it's a lie - the program only deals with 4:2:0

Use new YUV4MPEG2 API and add the y4m_accept_extensions(1) call.

Use y4m_si_get_{width,height} accessor functions instead of structure members
(which do not exist now in the new YUV4MPEG2 API).
