code cleanup/speedup
just to sync CVS with my harddiscs... :)

Somce nice C++ wrapper types for what were previously amorphous
C arrays or plain structs.

Fixed Dual prime selection to avoid illegal size vectors.
Simultaneously started replacing use of amorphous C-style
array element fiddling with use of geometric object classes

The usage() and manpage now (finally) agree with the code which puts a
-4.0 to 5.0 limit on -Q.

Make libmjpegutils a libtoolized shared library.

manpage and code disagreed on range for -Q, change manpage to agree with code.
The encoder used -q instead of -Q for the error message. Reword the manpage
paragraph for -Q slightly for clarity.

Typos & spelling errors, minor style/wording touchups.

fix for calling getopt 2 times. Now there is no need to set optind.

At least one hardware player is allergic to DPME - so update the manpage.

Add no-dualprime-mpeg2 flag and undocument no-altscan-mpeg2 flag
