Brought some changes into the howto I removed by committing my older version.
Also added a descriont for the -D option

-h used where -H was needed in mpeg2enc.1, -Q description reworded slightly to
make it more clear. AC3 audio works fine in mplex so remove the AC3 reference
in the BUGS section.

fixed some spelling mistakes.

Fixed bug in frame__YUV422_to_YUV420P() --- interlaced fields were not
being treated independently (so bottom-field-chroma was being pulled
out of the top-field).

o Updated the description of "-I" and "-z"
o Added notes to "-F", "-a": 'Taken from input stream header by default.'
o Added a couple of missing .PP's

Reinstate fieldenc-from-input-header patch (updated):

4:2:0 (vertical) chroma subsampling is dependent on interlacing vs.
progressive, and the MPEG decoder decides which to use based on
'progressive-frame' flag, and this flag is set/cleared in the MPEG
stream by 'fieldenc' parameter in the encoder library, so:

o Automagically determine 'fieldenc' (aka "-I n") from the input stream
header's interlacing info (allowing command-line arg to override).

Also:

o Declare "input_interlacing" as 'int', not 'unsigned int'.
o Initialize "input_interlacing" to Y4M_UNKNOWN.
o Promote unknown stream interlacing to 'progressive' for input_interlacing.

Sigh, forgot that lround() is in -lm.

lround() for systems which lack it (old glibc from 2.2.4 era and some of the
BSD variants).
