Further restructured low-level support and SIMD selection to
encapsulate it away from the main encoder.
Fixed nasty little source of non-determinist / compiler dependent
bahviour in the quantisation routines.
Remove references to the global syntax parameters from the lowelevel C / asm
routines.

Fixed a few old left-overs from earlier debugging that had turned off SIMD
quantisation on x86 machines.

Add new utility yuv4mpeg. Simple but useful program to add YUV4MPEG2 headers
to raw data (from ffmpeg) - see the comments in the source file for examples
of usage.

Remove unused variables, get rid of warnings by adding return type for main() and prototype for usage()

Continuing restructuring... fixed bug with INTRA being preferred
instead of skip coding for very low activity blocks.

simple whitespace edits in an attempt to get the public/anonymous CVS server up to date (it's 48+ hours behind)

get relative (negative) borders working.

speed up of ~6-7% if luminance contrast option is used. Use a table lookup
