Still wondering why opening the device segfaulted? Well, it's hard to open if the referenced structure is gone

At last... figured out how to get install-info working for Debian.
Bad bad Debian using a non-standard install-info...

Add special hack for dvdauthor in -f 8 to have hard-wired 18 byte headers even
if MPEG audio streams are present. One day I'll find out what a system
header *should* look like... (or dvdauthor will have a proper header parser).

Some fixes while testing under 2.5.x

Fixed the "read_header functions don't clear previous info first" problem:
o Two new public functions: y4m_clear_stream_info(), y4m_clear_frame_info().
o y4m_read_*_header() call these functions (respectively) at beginning.
o y4m_init_*_info() use these functions to set/init the default (non-)values.
Also:
o 'framelength' field in stream_info properly initialized.
o Moved y4m_fi_xtags() around to beautify source code.
