Fix compiler warning for unsigned < 0

Hopefully, tidied up some MSVC work-arounds, no real need but hopefully
these may work better. I vaguely recall some real pain with broken 64bit int's
in MSVC6. Sheesh, the more I remember/rediscover about this compiler the
worse it gets... P.O.S.
Fixed still overloading bug that broke run-out for stills multiplexing.

Increase version to 0.9.3 (forgot to upload this after the 0.9.3 release)

Fix missing check for channel<0 in VIDIOCGCHAN

Updated the "Optimizing the stream" added: "Scaling and offset correction" and "Frame rate conversion"
And some other small fixes

Good idea, couldn't be made to work. *severe* artifacts, and after a week
of trying 3 different methods all that was accomplished was slowing it down
about a factor of 5. Anyone wants it I have the sources of the 3 attempts.

Add some code inspired by some Iomega magic
