Fixed nasty buffer management bug...

Added bad last sector detection / warning useful for sensibly
handling truncated fields.

Cleaning up input bitstream class prior to abstracting away underlying
file I/O to allow arbitrary streams as read streams.

Fixed lack of default target still size for SVCD and
ommission of -T flag from man page.

Fixed a typping mistake.
Added 2 firms that build Picture optimizers (SIMA, ELV) to the Optimizing section.
Improved the manpage, mean the have the SEE ALSO section again.

Tidied up the general file structure and the object initialisation
sequence to produce cleaner encapsulation.
