Restructuring build directories to reflect decoupling of MPEG tools
from LAV IO libs. First stage in making the two components seperately
buildable / installable (transcoding apps won't need lavtools).

Gah! VCD audio problem found! VCD seems to insist on leaving
the last 20 bytes of each audio packet null. Which cretin was
responsible for *that* one I wonder...

Removed incorrect handling of multi-AU packets.
Fixed PTS/DTS calculations and added test stuff to track down
audio mux-ing bug.
