More VCD modifications. Seperate sys header sectors for audio and video.
SCR adjusted for null sectors inserted before the actuall MPEG stream on the
disk.

Tricky VCD fix. It seems the VCD bit-rate calculations include parts of the CD sector headers as well so you need to include these in the SCR and hence
PTS/DTS timestamp calculations. Also, for some reason they don't bother
including video buffer information in the system header.

All fixed now. Here's hoping this finally fixes it.
