Avifile's changed DestFmt () to GetDestFmt () so we had to, too. This means
old versions of avifile may no longer compile.

Fix breakage caused by editlist/LavParam changes.

Set variable GOP size by default for VCD
Change GOP splitting algorithm to eliminate some annoying characteristics

Change the luminance_mean algorithm to sample only a subset (for speed)
and include chroma.
