It shouldn't be necessary to add an entry for etex, it is already
present in a default installation.

Well as you didn't say exactly how you did install "2.9 over 2.7"
and how you did add a etex in the formats did when you added the
button one can only guess. 

I would suspect that remains of the older miktex (older formats, an
older formats.ini, or environments variable) confuse the newer one.

I doubt it. But if you are the only miktex user on your PC I advise
you to install next time directly in your user account in single
mode. With a multiuser installation it is important to understand
when to manage the system as admin and when as user and how this
modes can interfere. As you have rebuild the formats in user mode,
you now have local formats files which will takes precendence over
the format files generated in admin mode.
