Thank you for replying.

By a stroke of luck, I have found out what was causing the 
problem.  I have been using MathTime fonts in my document along with 
a package called mathtimy.sty.  That package appears to be out of 
date.  If I use package mathtime.sty instead, which appears to be the 
version supported by MiKTeX, the problem disappears.
