Though everything appears fine in PS file. The shifted top margin is because Ghostscript (which is the enginge behind) ps2pdf has a different default page size than you. Either give the papersize as an option to ps2pdf (read about the Ghostscript options) or if you always use the same papersize, change it in texmf\ghostscript\base\gs_init.ps Look for the line "% Optionally choose a default paper size other than U.S. letter." and remove the percent sign in front of the following line if you want a4.
