Well, the "revtex4" package only has revtex4.cls with that name. If you installed it using the MiKTeX Options Wizard or Package Manager it should be C:\TeX\texmf\tex\latex\revtex4\revtex4.cls If it isn't there, find it and put it there (together with the other files) and refresh the filename database (MiKTeX Options Wizard).
