#15 duplicate numbers in 'starts at' list

When adding files to the list, the "file extension starts at" list can get to contain the same number twice:

1. start KRename
2. add the file "1...5"
-- we now have 1, 2 and 3 in the "starts at" list --
3. add the file "1...5....0....5....0....5....0....5"
-- we now have two instances of "2" in the "starts at" list, and they behave differently --
4. add the file "1.....2.....3.....4.....5.....6.....7"
-- we now have lots of duplicates in the "starts at" list, and they behave differently --
