I'm sure this is because of some wierd setting I have on my Windows desktop, but I just tried to add an Auto-type entry, and it appears that perhaps my font is too large, and the modal dialog occludes the "enter" or "accept" button.
In the image, the "placeholder" text is truncated on the right. There appears to be a button that is reachable on the left (arrow), but that button just takes me to a help screen. Since its modal, I can't resize it.
It's been a while since I used the auto-type set-up (damn program just works without a whole lot of futzing around) but I'd guess that the accept button is occluded by the "placeholder" help info.
KeePass dialogs use Windows System fonts. After some Windows text size settings changes you should reboot after adjusting, and the accessibility text size adjustment settings should be avoided. See https://keepass.info/help/base/faq_tech.html#guifont for details.

Edit: Nevermind... I had MS San Serif 11 set in KeeUIExt. Disabling that fixed it so that there is now a visible scroll bar on the help dialog, and the OK and Cancel options are now visible.

Rebooting didn't help.
I have a 17" laptop with a 2560x1440 display. I also have failing eyesight.
Per the help page above, I adjusted my scale from the recommended 150% to 125% and rebooted, but although everything got dramatically smaller, the proportions didn't change, so the panel was still occluded.
Under "Accessibility > Text size" (which I think is the "Windows Settings → 'Ease of Access' → 'Display'" listed on the docs page - Windows 11 Pro, BTW) the text size slider is set to 100%. It doesn't appear any accessibility settings have been adjusted.
I didn't see the "Windows Settings → 'Accessibility' → 'Vision'" section mentioned in the docs above (probably renamed at some point), but I looked through all the accessibility settings and didn't find anything set to a non-default value. Perhaps its in a new place? At any rate, I don't recall changing something like that.
I have the "Notes Font Selector" plug-in installed, but it's not enabled. I have several other GUI-related plug-ins. I don't think they are relevant, but I could be wrong... see below.
So, just to double check... normally the buttons aren't occluded, I assume. Is there a screen cap laying around of how it should look? Perhaps that would yield some clues.

The Custom user interface font setting allows for very aggressive font adjustments but doesn't adjust all KeePass interfaces uniformly. I was able to recover from a font adjustment to 16pt by temporarily reducing my Windows Display Scale setting so that I could uncheck the KeeUIExt Custom user interface font setting, save the setting (press the OK button), and restart KeePass.
I suggest you use this KeeUIExt setting very sparingly and rely on the Windows Display Scale setting instead.

I've now improved the custom font support in the auto-type association dialog, such that it's now fully compatible with the KeeUIExt plugin (I haven't tested the other plugins).
One note: It looks like it might break the KeeTheme integration, but I think you suspected that might be the case.
I did manage to find an edge case, but its pretty much the same as mentioned above - modal dialogs can be a problem.
I tried increasing the KeeUIExt font size to an unreasonable 20 points. I think it worked, but on my 2560x1440x150% screen, the dialog was too big to fit on the screen. Since the title bar was kept on screen, the okay/cancel buttons were off-screen (physically off-screen, not the original obfuscation issue), both for the Edit Autotype, but also for the KeeUIExt dialog.
Of course, that's a Windows issue, not a KeePass one.
When I opened KeeUIExt, I could see theKeeUIExt alternate font toggle on screen (if I'd gone bigger than 20 points I might not have), and I toggled it off. Fortunately, I was able to blindly tab offscreen to the correct button on the KeeUIExt dialog to accept the change, and almost everything resized and was well again without even restarting (or rebooting!).
I did note that the "File-Group-Entry-Find..." strip under the title bar had sized up when I increased the font, but didn't "shrink" back when I disabled the alternate font, although everything else did. But by just going back to KeeUIExt (which now fit okay) and toggling the alternate font back on to the default (MS San Serif 10 pt) that fixed the command strip as well! Again, no restart, no reboot!
I'm sure that making dialogs resizable would be virtually impossible, but it dawns on me after using Windows for many, many years, it would make a LOT MORE SENSE to put the Okay/Cancel, etc. buttons at the TOP of the dialog, near the title bar, instead of at the bottom (at least for anything that modifies the UI or accessibility) so they are almost guaranteed to be on-screen.
Even so, it might be possible to change to a font that would even push those buttons off screen, but the font would have to be so big it would probably happen a lot less often, and accidentally rather than on purpose.
Alternatively, I've seen in some applications recently a scroll-bar in the settings dialog, such that the contents can be too big to fit on screen, but you can scroll through the entire contents, including the okay/cancel buttons.

Have you tried rebooting?
